The Consent and Authorization form is like your permission slip! It must be completed and signed by the parent/guardian AND student, EVEN IF THE STUDENT IS 18. Turn into the Senior Table at Registration. If it is after registration, then turn it in to Oak Park High School's main office. Please put it in an envelope marked GRAD NIGHT. Notate the student's name. Only checks are accepted. NO CASH!

Payment MUST be made by check only.
Make your check payable to OPHS PFA and make sure your student's name and GRAD NIGHT are noted in the memo section. Attach your check to the completed permission slip and turn it in to the office at OPHS. DO NOT TURN IN TO THE STUDENT STORE.

If you need financial assistance please see your counselor for further information. Financial assistance can't be approved by the Grad Night committee - it must be approved by the OPHS Counselor. Counselors are listed below.
